DR KENDALL WILL BE RETIRING AS OF FRIDAY 23RD OF FEBUARY 2024
We would appreciate your cooperation in ease of accessing and transferring your medical records to a new GP or medical practice.
Patients will need to confirm that your new GP is taking new patients and complete their Request for Transfer of Medical Records Form for yourself, plus or minus, your family members with their consent/signature if over 18.
The practice will email the form to us at wembleygp@westnet.com.au
We do charge a small fee for transfer of records. If you would prefer we can upload records to a disc or USB for you to collect and take directly to your new doctor. We will not be printing any paper records for both privacy and environmental reasons.
Requests for picking up records from the practice must take place before end of business on the 23rd of February, after this date all records will be going into secure storage.
Records will still be able to be requested after our closure on the 23rd of February.
Requests can be made for your record transfer via our email address as it will be monitored on a weekly basis.

PRACTICE POLICIES : WE ARE NOT ACCEPTING NEW PATIENTS AT THIS TIME.
To protect the health and safety of our staff, patients and their families ALL patients attending the practice must be at minimum double vaccinated against COVID.
You may be asked to provide proof of COVID vaccinations when booking or attending appointments.
If you have any cold and flu like symptoms including sore throat, cough, fever, runny nose etc please let us know when booking your appointment. We will advise you on how to proceed with attending your appointment
You may be required to provide a negative RAT or to wait in your car and call reception before entering the practice
If you can't find a suitable appointment time, or are wishing to book nurse appointments including childhood immunisations, flu vaccinations or COVID-19 vaccinations then please contact reception to make your appointment.

As a private billing practice, we strive to provide the highest quality services for our patients. Practice fees include $85 for a standard consultation and $145 for a long consultation, with a Medicare rebate of $39.75 and $76.95 , respectively.
